We understand that artists of The Penguin Project have a variety of disabilities. We have made an attempt to create an application that includes all students with disabilities. Please answer each question carefully and as completely as possible. If you are unable to complete the application on your own, you may have someone assist you, and they will need to verify their information in the Submission and Certification section.
If there is a question that does not apply to you, mark N/A in the section.
